The Canby Family Practice Clinic is accepting new patients. We are not a walk in clinic, please call our office to schedule appointments for Dental or Medical Depts.

At the Canby Family Practice clinic, our front desk staff is required to ask all patients about their insurance so we will know, how best to serve you. We are required to check all eligibility on Medi-Cal patients attached to Partnership Health Plan of California before scheduling any appointments.
In order for our Providers at the Canby Family Practice Clinic to provide you with the best care regarding your medical concerns and conditions they request to see all Medical records. We can mail or e-mail the records release forms to you and mail them to our office.
Our providers request that new medical patients, bring all medications to their first appointment at the Canby Family Practice clinic.
Our providers are dedicated to children and preventive medicine. We will see children without records or a medical history form. Our providers request that you bring all Immunization records with you to the first appointment for your child.
All new patients will be confirmed to come early for their first appointment at the Canby Clinic so the front desk staff can have you fill out paperwork for your chart and present Insurance information photo ID and co-payments.
Sliding Scale Fees:
Sliding scale fees are based on family size and income. This program offers anywhere from a 10% discount to a 50% discount. We need proof of Income for one month from all members in your household that receive income, to determine eligibility. Application form available on the “Downloadable Forms”. Payment is required at the time of service to receive the discount. No-cost services are available for well-child health exams and family planning. Other grant funds may be available as well. Please check with our receptionist if you think you might be eligible for any of these discounts. Patients who do not qualify for the discount program and remain on self pay will be offered a 10% discount for payment in full at the time of service.
We participate with many insurance plans, including Medi-Cal, Medicare, Blue Cross, Blue Shield and Delta Dental. If your insurance covers treatment, you will receive the benefit of reduced personal costs. If your Insurance has a Co-pay we will require that you pay it at the time of service. Insurance policies vary, so we will review your insurance to determine the appropriate course of action. Once treatment has started, we will file your claims. If you have a question about what Insurances our office will accept please feel free to call our office and talk to the Insurance Specialist. We will do our best to become a contracted provider with your insurance if this is possible.
To help us with your insurance filing, please complete the insurance questions presented to you at the time of your appointment. Please remember that insurance is a benefit for you or your family. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges. We request that you let our office know of any changes with your insurance as soon as they take place or at your next scheduled appointment.
Special Notice about Partnership Health Plan of California:
Partnership Health plan of California is a Medi-Cal health system under contract with the State of California also known as Managed Med-Cal. Eligible Medi-Cal members will be sent a letter from Partnership welcoming them to the program with paperwork for them to fill out choosing a Primary Care Provider clinic or medical group. If patients do not choose a Primary Care Provider then Partnership will select one for them. The front office staff at the Canby Family Practice clinic is required to check the eligibility status of each Medi-Cal patient that chooses to be seen at our clinic. We will inform you if your partnership information states that you are set up under a different provider or clinic. To change your Primary care provider you can contact Partnership at 1-800-863-4155 they have live operators to assist you every Monday-Friday from 8:00am – 5:00pm.
